Health and Wellbeing

There are many things in a person’s life that can impact their wellbeing and, in turn, their health. Whilst the symptoms can sometimes be treatable by medication, often the root of the issue is something that cannot be treated medically. This is where our Social Prescribing Link Workers can step in. They can work with you on a one-to-one basis to find out what really “matters to you” and support you through the process of making positive changes, as well as signposting to other organisations that may be able to help.

Some of the areas that our Social Prescribing Link Workers can support with include:

  • Carers and Care Support Services
  • Housing, Benefits and Financial Advice
  • Employment, Training and Volunteering
  • Education and Learning
  • Creative Activities e.g Gardening, Art
  • Befriending / Local Social Groups
  • Counselling Services
  • Support with Smoking
  • Support with Alcohol / Drugs
  • Weight Management
  • Physical Activity

Your Social Prescribing Link Workers are Jenny Friend and Gemma Finan.

Jenny started her told as a Social Link Worker in October 2019 and has seen the service grow and develop from day 1.

“One of the best things about this role is that every call is individual – you just never know what you will be asked next!”

Gemma joined the team and begun her role as a Social Prescribing Link Worker in May 2022.

“I really enjoy getting to meet different people within the community and helping them to achieve their goals.”

As well as supporting individuals on a one-to-one basis, Jenny and Gemma also run projects to help support and improve the wellbeing of people within our local community. This has included:

  • Mini First Aid sessions to new parents
  • A drop-in session at The Well in Kibworth on the 1st Thursday of the month
  • Well-being walks
  • Transporting isolated patients in Great Glen to local craft and wellbeing sessions
  • Working in partnership with Harborough District Council and Everyone Active to develop a community garden.

How do I speak to someone from the team?

You can speak to someone at your GP practice and ask for a referral to the Health and Wellbeing Team. Or you can self-refer via email: [email protected].

Patient Feedback

  • I learned about applying for appropriate services to help me and financial help too as well as outside contacts and groups that could help too. The social prescriber was kind and had a lovely manner which was sympathetic and supportive. Felt cared for not just a patient to be felt with. I felt heard and cared for.
  • A follow up email was provided with sufficient information and links that gave me multiple options. This was great as I was able to choose what worked best for me and felt more personalised to my needs.
